Survey: UML and Diagrams

I am soliciting contributions for a 15 question research survey ( http://www.questionpro.com/akira/TakeSurvey?id=981322 ) regarding the impressions and experiences of software professionals using diagrams and UML. The results will be made publicly available through my blog at DobbsCodeTalk.com ( http://www.dobbscodetalk.com/index.php?blogger=cdiggins ). The contributions of JoS readers would be much appreciated.

The questions are rather vague.  Do you use it?  What skill level (beginner, intermediate, expert)?  Which diagram do you use the most?

And text boxes for answers?  How are you going to collate all these different sentences?

The questions were open-ended and somewhat vague, because we are trying to get a baseline for what professional software developers think of diagrams and modeling. This is to help us to direct our research.
