Can anyone recommend a good free or open source utility or framework for end-user documentation? I'm not looking for anything fancy, but would prefer not to start from scratch.
Basic requirements are:
1) a table of contents that allows topic depth of at least three levels.
2) table of contents shows at all times, with current selected topic highlighted.
3) can be generated static content or dynamic using PHP.
4) built in support for internationalization would be nice, but not required.
Basically, I want to be able to cut and past topic titles and text to the appropriate places, and have everything else already set up. I'm sure something has to be out there, but I haven't come up with much yet (search terms for something like this are pretty vague.)

You didn't specify web or paper formats but doxygen will do both for you. http://www.doxygen.org
