generally I use UML in a conceptual manner. It helps me to better think about problems, 'cause with visual artifacts, colors, assosiation, my mind is able to recognize the main aspects of a problem.
Useless to say that there are a lot of techniques to improve a UML diagrams. The Better is diagram the less I need to "study" it.
Tools: I use a bunch of tools, dependes upon the project size.
Fujaba, BOUML, Enterprise Architect, Poseidon,...
