Everything looks good (and the original was way too plain) but I'd avoid using the word "payment". The new form has it twice.

Talk about what they're getting, not what they're paying.

But yeah, tis good.

Nice case study. The before and after is striking. Before doesn't look like the normal payment page, kind of dodgy. After looks like the normal payment page found at big name sites. I hesitated at the before image. Hm, is this a legit site, or is this dude just stealing CC numbers? The after looks legit. Of course looks don't guarantee anything at all. But they do convey information. Want to look legit? Don't look fly by night.
